Measure temperature and assess symptoms prior to visit:
- Complete screening form and email clinic prior to visit
- Infrared Thermometer used on patient entry to clinic
- Any determination of symptoms may terminate visit

- Persistent pain or pressure in the chest
- New confusion
- Bluish lips or face
If you are Symptomatic
- Please return your screening form prior to your visit
- Admin team will share your screening responses with physician for determination if able to provide care at in person at clinic
- Might require a brief phone consultation prior to appointment
- If determined not safe for in person care at clinic, please contact your PCP for evaluation and possible COVID 19 testing
- Our clinic will schedule a tele psych appointment within in 48 hours to ensure coordination of care and adequate supply of medications for anticipated quarantine
Disinfect and Clean Clinic
Hand Hygiene (>20 seconds)
- Before and after each patient exposure
- Before and after cleaning of surfaces
- Before and after donning of mask and PPE
Cleaning of Surfaces and Devices
- Clean TMS device and Ketamine chair after every patient
- Clean table, door knob, pens and other surfaces after every patient
- Clean community areas (bathroom and waiting room) 4x per day
Other Safety Efforts at Our Clinic
- Replaced lobby’s water cooler station with hand sanitizer station
- Reduced touching of required documents
- Calling/emailing to confirm appointment and authorize payment
- Sending/scanning required forms prior to appointment
- Expanded appointment blocks to incorporate time for cleaning
- Staggered patient appointments to reduce waiting room occupancy
- No more than 2 patients in waiting room (ex. 1:15pm start versus 1pm

High:
Prolonged close unprotected exposure to an aerosolizing patient
Medium:
Prolonged close protected exposure to an aerosolizing patient
Low:
Brief protected or prolonged close contact while both in mask
Our outpatient clinic is LOW risk:
- Close contact is generally brief and not face to face
- Both patient and care giver are wearing masks
- Patient and staff are screened each day
